TRANSFER OF RESPONSIBILITY DURING ANAESTHESIA

It is recognised that it is sometimes necessary to hand-over of responsibility from one anaesthetist to another during an anaesthetic. This may be on a temporary relieving basis or on permanent basis.

1. A person asked to take over an anaesthetic should decline if they believe the case is beyond their expertise. The primary anaesthetist must be satisfied of the competence of the relieving anaesthetist to provide care.

2. Prior to the transfer of care both anaesthetists should ensure that the following information is conveyed and observations checked.

  1. Patient History

    Pre-existing disease

    Present medication

    Allergies

  2. Nature of the operation.

  3. Anaesthetic management

    GA/Regional, Local Block, Sedation

    Drugs, amount/time (all syringes labelled)

    Fluid status and therapy. Blood availability, if necessary.

    Problems current and anticipated.

    Throat pack

    Anaesthetic record complete to date

  4. Patient Observations

    Airway and ventilation including correct placement of ET tube

    Heart rate and blood pressure

    Colour and SaO2

    Eye protection and patient positioning satisfactory

    Condition stable

  5. Anaesthetic Machine Check

    Reserve oxygen

    Gas flows

    Volatile agent

    Circuit

  6. Monitors and ancillary equipment check

    Functioning correctly and alarms set

  7. Surgeon informed

3. Both anaesthetists must be satisfied with condition of patient and agree that transfer appropriate. Notation that transfer has occurred with the time should be recorded on the anaesthetic record.
